Registration can now be completed by filling out THIS FORM.
Please complete this form and bring with you to the Crown Promenade Hotel. Upon arrival at the venue, please proceed to the Conference Secretariat (M5, level 1) where you registration can be processed. Payment may be made by cash, Visa, MasterCard or AMEX. Cheques may be made out to ICE Australia.

Member Rate
To be eligible for the member's rate you must be a member of the GDAA. You will be asked for your membership number so please have this ready when registering.
Early bird Rate
To receive the early bird rate you must register online and pay by credit card at the time of booking. Early bird registration closes on Thursday 29 October. Please call the Conference Organisers on 03 9681 6288 if you have any queries.
Student Registration
To be eligible for the student rate you must be a full time student. Please provide proof of your student status along with your registration form. A photocopy of your student identification including full time status will be sufficient. This can be faxed to +61 3 9681 6653.
The deadline for all registrations is Wednesday 25 November 2009.
Registrations made after this date will incur a $AUD50.00 late registration fee.
Payment can be made by credit card (MasterCard or Visa) or cheque in Australian Dollars.

Cancellations & Amendments
Registration and/or accommodation cancellations and amendments must be sent in writing either by mail, fax or email to ICE Australia Pty Ltd.
Registration cancellations received up to and including Friday 6 November 2009 will receive a full refund, less AU$175.00 administration fee. Registration cancellations received after Friday 6 November 2009 and up to Friday 20 November 2009 will receive a 50% refund (minimum AU$175). No refunds will be given for registration cancellations received after Friday 20 November 2009. As an alternative to cancellation, your registration may be transferred to another person up to 15 days prior without receiving any extra cost penalty. Transfers made 14 days prior and less will incur an AUD $20.00 transfer fee.
Accommodation will not be reserved without credit card details. Accommodation cancellations received after Friday 6 November 2009 will be charged by the hotel according to their cancellation conditions.
ICE Australia reserves the right to deny entry and/or terminate the registration of any person who demonstrates behaviour that is deemed by ICE Australia to be inappropriate and/or a risk to others. |
